I authorize NEWORKX and its agents to share and receive my personal and program information (profile, training history, enrollments, job applications, certificates, progress, and compliance notifications) with the referring court, probation office, training providers, workforce agencies, employers, and authorized partners to administer services, verify compliance, and support placement. This authorization is effective until I revoke it in writing; revocation won't affect prior disclosures.
